The Death of the Data Code of Honor
For most of modern medicine, data governance has been an article of faith. Institutions promised to “protect privacy” and “ensure ethical use” — but those promises were procedural, not structural. Ethics lived in policy manuals, not in code.
As data volume and velocity exploded, good intentions failed at scale. Consent became unreadable, privacy untraceable, and accountability fragmented across vendors. Governance by paperwork collapsed under the weight of automation.
The next generation of trust cannot depend on belief; it must depend on architecture.
